1. What we collect
Your name and contact details, your photo ID, the answers you give in your intake, documents you upload, notes and summaries from your appointments, and a record of your payments. Card numbers go straight to our payment provider; we keep only the last four digits.
2. How we use it
To provide and coordinate your care, to prepare your clinician before each visit, to send your care pathway and other records, to take payment, and to meet our legal obligations.
3. Your health information
Your health information is protected under HIPAA. Our full Notice of Privacy Practices, which you sign before booking, explains your rights in detail.
4. Who we share it with
Only people involved in your care at Foreviva, and the services we rely on to run it — our payment provider, video provider and secure hosting — under agreements that protect your information. We never sell your information.
5. Call recordings and transcripts
Video appointments may be transcribed so your clinician can write up the visit. The recording is discarded by the provider once the transcript is produced, and the transcript is kept with your record.
6. How long we keep it
Medical records are kept for as long as the law requires. Anything else is deleted when it is no longer needed.
7. Your rights
You can ask for a copy of your records, ask us to correct something, or ask how your information has been used. Send us a message from the portal and we will reply within 30 days.
