1. Who we are

Foreviva Medical Clinique is a specialist practice for endometriosis and pelvic pain, with a clinic in Menlo Park and video appointments. These terms apply whenever you book, attend or use your Foreviva account.

2. How care works here

Your first step is a pre-screen with a care coordinator, who reviews your intake with you. Your consultation with Dr. Joelle Osias follows, and is confirmed once the pre-screen has been written up. After it you receive a care pathway setting out what happens next.

3. Fees and payment

Foreviva is a cash-pay practice and does not bill insurance directly. A one-time $150 deposit is taken when you book and covers your $150 pre-screen. Your $350 consultation is charged automatically to the card you paid the deposit with as soon as it is confirmed — you are not asked to pay it separately. A Good Faith Estimate of expected charges is issued before your visit, and a superbill is available on request.

4. Cancelling or moving an appointment

You can cancel or move an appointment free of charge up to 24 hours before it starts, and anything taken for it goes back onto your deposit. Within 24 hours, or if you do not attend, the fee is kept.

5. Video appointments

Join from a private, quiet place using the link in your portal. The link opens 10 minutes before the start. If the connection fails, your clinician will call the number on file.

6. Your responsibilities

Please give complete and accurate information in your intake and keep your contact details up to date. Tell us about any change in your symptoms or medication before your visit.

7. Emergencies

The portal and messages are not monitored around the clock. If you have an emergency, call 911 or go to your nearest emergency department.

8. Changes to these terms

We may update these terms. If anything material changes we will tell you in the portal before it applies to you.
